length of training (none longer than neurosurgery)
competition (neurosurgery attracts some of the top students in medical school - applying for a small number of residency slots in large numbers)
work load (I don’t know a neurosurgeon who works less than 50–60 hours a week, and many work much longer)
how demanding the work is (how far to resect a tumor from an eloquent part of the brain is a lot more stressful)
